Sense Lang has a very thorough, excellent online touch typing course you can go through. It even allows you to choose between QWERTY and Dvorak layout.

Remove the tempatation to look at the keyboard. Take some small stickers, or just the sticky part of post-it notes, and cut them up, and cover the alphanumeric keys on the keyboard. This will remove the temptation to look down at the keyboard, because it will be futile, there will be nothing to see.

EDIT: This will help with tip 1 on the site that John T suggests.

I did this when I was 11, my cousin was doing a secratarial course and had covered over the keys on her typewriter to learn to type, and I remember thinking (even though it was 26 years ago) "well, if she can do it, so can I" so I went home and did the same thing to my dad's typewriter.

You'll take a hit in your accuracy and speed for a short while, but as you're teaching your muscles to find the right keys eventually you will type words without having to stop and think where the keys are, you will just think of the words, and your fingers will dance across the keyboard like Fred and Ginger.
