Many times the manufacturer of the label and inserts will provide templates for one or more graphics applications; you should consider using one of those if you want a fully-custom print.

If you want to use open source software you should consider cd.cls which produces the following results. This solution uses the typesetting software LaTeX and should be highly customizable.
