Is there some online tool which lets me design a tree structured questionaire/survey which suggests a solution to a problem?

Example troubleshooting survey for the problem of black monitor screen:

Q1: Is the powercable connected to the monitor?

  • yes
  • no

If answer is no: "The solution to your problem is to connect the power cable"

If answer is yes: Continue with Q2

Q2: Is the computer turned on?

  • yes
  • no

Is there something like Google Forms, where I can achieve this?
