I am looking for a way to conduct online surveys within my organization. I am looking for a tool that is

  1. Free or Open Source.
  2. Accumulates responses automatically in a Microsoft Excel or CSV file.
  3. Does not need extensive back-end installation.

I am aware of the surveys feature in Microsoft SharePoint. I am trying to find a way not to use SharePoint :-)

LimeSurvey fits the first two requirements, but does require a certain amount of back-end set-up. However, if you were to use a pre-built appliance (eg the one at 9while9.com) then you'd have minimal set-up too.
