Are there any Mac OS X apps that support n-up printing of duplicate and arbitrary pages of one PDF to another PDF?

Suppose I have a multi-page PDF and want to do 16-up printing. But instead of printing all the pages in the master document or a range of pages I want to print an arbitrary number of pages with some duplicates. For example, 16-up printing of pages: 1,1,1,1,2,2,4,16,32,24,24,24,34,34,35,99.

The default print-to-PDF functionality in OS X and Preview only allow printing to a page range. You can select arbitrary pages to print in Preview, but cannot print the same page multiple times. Acrobat Reader has a free-form page range box but doesn't appear to allow duplicate page entries.
