Anyone know of any browser based software that allows you to build a web form. We need something that allows you to place controls with absolute position, mostly text boxes but some dropdowns as well.

After the form is saved it would be accessible just like any other web form.

The point is to allow the user to design a form that they can fill out and then have it print on top of the specific paper form (official goverment issued form).

Ideally we want something open source so that we can extend it to suit our needs.

Why not just have a specific form created for the user, rather than having them create a new form each time? I would think that it would be rather unpredictable in what types of data you would be getting from many different custom forms. – Chris Dec 31 '09 at 20:00
While every form is going to have the same items on it in roughly the same position, every printer is going to print slightly differently. Since this is going to be printing on specific paper forms we have to allow the user to make the slight adjustments necessary to line things up. I may not be explaining it well but think of a HICFA medical form. It has to be filled out on their form, you can't print your own so your choices are to fill it out by hand, use a type writer where you can move the paper around exactly or do this to line things up to the .001 inches. – invertedSpear Dec 31 '09 at 20:30

Wavemaker might be what you're looking for, although it may be overkill for just making forms.

It's open source under the Apache License.

Thanks, but you're right, way overkill. Just looking for something similar to the web form builder included in MS Access. I need to be able to design the form, position the controls exactly, then tie it to data (prob XML). Visual Studio is great, but it's gotta be an in browser tool. – invertedSpear Dec 31 '09 at 19:05

Check out formgenics. The site looks pretty ghastly, but it does provide a very simple browser based form creator.

Thanks, but that doesn't allow you to position the controls. It would be great if we just needed a form made, but It doesn't allow the user to place things exactly where they are needed. – invertedSpear Dec 31 '09 at 20:34
