You're VERY confused about Fiddler. Fiddler is a web debugging tool. It has no malicious behavior whatsoever, and it is never installed unless you personally install it using the installer downloaded from Telerik. The scenario described here is a piece of malware which is attempting to avoid detection by making itself look like Fiddler.

FWIW, later versions of Firefox do adopt the "system proxy" setting by default. See telerik.com/blogs/configuring-firefox-for-fiddler for details.

No, Internet Explorer doesn't care, at all, about the blank. You can tweak the header with Fiddler for any site and see that this is the case. You can also return the exact response using Fiddler's AutoResponder and find that it works. The most likely explanation is that he failed to properly disable Enhanced Protected Mode, which results in the exact behaviors described with and without Fiddler.

IE has always sent a HEAD request when downloading content for an object tag if the type of that Object is not known. Specify a Type attribute on the object to avoid needing that. Keep in mind that the PDF handler may itself make a HEAD request too.
