Run Process Monitor and see why it is failing to execute; I am guessing the program requires elevation, and these will not run during Windows startup since it would be annoying to get prompted there.

Windows is case-sensitive but the file is still on the DVD. You could try something like IsoBuster (or any tool that works with DVDs or ISO images). If it can't browse to the DVD and copy an individual file you could just create an ISO image and extract the file with any program that can open ISO images.

Actually multiple monitors are not common at all (especially in the consumer space); if you believe Microsoft's numbers only 15% of desktops have multiple monitors. Most of those people are either professionals (developers, artists, etc.) or enthusiasts whose workflow will most likely not include Metro apps anyway.

Everything I've read indicates that Metro apps can only run on one monitor at a time; the other monitors will show the desktop. Start screen can be brought up on any monitor, but the others will get switched back to the desktop.

Because the filesystem is NTFS, and ACLs are NOT optional. If you don't want to deal with ACLs at all you could reformat it as FAT32, but you would lose some functionality (the most obvious being limited to a maximum 4GB file size).
