I just tried: if I run phpunit outside of IDE (from command line) and generate coverage.xml file there, I can then attach it to IDE using the 3rd step with no issues. Possibly it works for you (if you have correct paths) .. but default coverage colors are too faded/light and you just cannot see them? (by default only small area in editor gutter is highlighted)

I've used this when this functionality was just introduced in PhpStorm (v4 or 5) and built-in coverage was not working properly (due to a specific bug with file locations). The only difference to what I did before is the file paths.

Use any Windows 7 tweaker tool -- it should have such option available (Yamicsoft Windows 7 definitely can do this). You can hide such accounts using secpol.msc (then "Security Settings | Local Policies | User Rights Assignment | Deny log on locally"), but it is not available for Home Premium. Other than that -- you will have to edit registry directly.

@andrecarlucci Yes, it's empty -- this means no auditing is setup (that is the default for almost all files except maybe very few system files -- auditing can produce noticeable overhead on very busy systems). Click "Edit" and then "Add" to create auditing entry/rule.

For Windows 7 look at "Administrative Tools | Local Security Policy | Security Settings | Local Policies | Audit Policy" (or just run secpol.msc). For files/folders also look at "file/folder | Properties | Security | Advanced | Auditing"

You can use one of the Backup programs (trial mode should do just fine) to backup these settings on one PC and then restore on another (passwords are encrypted using specific UserID, so they will not be restored on another PC (Outlook will ask for a password again), unless program knows how to decrypt them). I did it using KLS Backup. I'm sure there are other programs that can do the same.

This line on it's own make little sense (most likely you will have 500 server error if it would be only one rewrite line). There has to be one or more RewriteCond lines before this one.
